How this website handles your information

The call-back form collects your name, phone number, email address, who you are, and a general topic. It does not collect medical information, and we ask you not to type any. The form sends a text alert to our staff and stores the request in a private spreadsheet owned by the clinic. We use it only to call you back.

Medical records are never collected through this website. Once you are a patient, your health information is handled under HIPAA through the clinic's own systems.
